Organizations may not receive a grant in two consecutive years.
Organizations must be designated as a 501(c)(3) entity by the Internal Revenue Service.
Funds must go to the organization directly and not pass through to any other organization.
Organizations may only apply to the SIP programs located in the city in which they provide services (example: an organization that provides services only to Midland residents can not apply to the SIP programs located in Odessa).
You must complete your request on the grant application forms provided below. DO NOT use "See Attached" as a substitute for completing the form.
Please check with the individual campus to determine deadline dates and number of copies required.
Because each campus requires multiple copies of the grant application forms, do not email the forms. You must submit the application by mail.
The following forms are Microsoft Word documents. The forms are protected to only allow input in the spaces provided. Financial forms include automatic calculations. Answer all the questions on the forms provided and do not use "See Attached" to refer to attached documents in lieu of completing the forms.
PLEASE NOTE: Depending on your browser and computer settings you may need to open each form and save them to your hard drive with the Microsoft Word extension (ie: CoverPage.doc or CoverPage.docx" depending on your version of Word). Once saved close your browser and use the saved file for input.
